The NDIS Terms of Services is between a participant of the NDIS (National Disability Insurance Scheme) including their guardian and plan nominee and the provider Home Therapy Australia PTY LTD.
Terms of Service
Service agreements outline the terms of service which are provided for the purpose of providing supports under the participants NDIS plan.
Parties accept that these terms of service are constructed with regards to the NDIS of which aims to:

- support the independence and social and economic participation of people with disability
- provide reasonable and necessary supports, including early intervention supports, for participants in the NDIS
- enable people with disability to exercise choice and control in the pursuit of their goals and the planning and delivery of their supports
- promote the provision of high quality and innovative supports that enable people with disability to maximise independent lifestyles and full inclusion in the community
Service Provision
Home Therapy Australia PTY LTD agrees to provide the participant with allied health services for the period stated in the Participant's NDIS plan.
Home Therapy Australia PTY LTD agrees to:
- Review the delivery of supports annually at the very least, with participant.
- Provide supports that meet the participant's needs and preferences once agreed.
- Engage in open and honest communication in a timely manner.
- Treat the participant with care, courtesy and respect.
- Discuss with the participant regarding how supports are provided.
- Provide the participant information regarding managing any complaints or disagreements and providing details of Home Therapy Australia's cancellation policy.
- Listen to and quickly resolve participant's feedback and problems respectively.
- Protect the participant's privacy and confidential information
- Provide supports in a manner that is consistent with all relevant law which is inclusive of; Australian Consumer Law and the NDIS Act 2013 and rules and keep accurate records of the supports that are provided to the participants
- Issue regular invoices for the supports provided to participants.
The Participant/ Participant's parent or guardian agrees to:
- Inform Home Therapy Australia PTY LTD regarding their preferences regarding support delivery and needs.
- Treat Home Therapy Australia PTY LTD and their staff with courtesy and respect.
- Communicate with Home Therapy Australia PTY LTD if the participant has any concerns or questions regarding the supports being provided.
- Provide Home Therapy Australia with a minimum 48 hours notice if the participant can not attend a scheduled appointment. If the notice is not provided by then, the Home Therapy Australia PTY LTD cancellation policy will apply.
- Provide Home Therapy Australia PTY LTD with the required notice if the participants need to end the service agreement.
- Notify Home Therapy Australia PTY LTD immediately if the participant's NDIS plan is suspended or replaced by a new NDIS plan, or the participant stops being a participant of the NDIS.
Schedule of Supports
Supports provided by Home Therapy Australia PTY LTD and pricing is discussed and agreed at the initial assessment with the therapists directly. These budgets are able to be processed, updated and modified as the plan progresses between the participant and Home Therapy Australia PTY LTD.
Home Therapy Australia PTY LTD charges. under the NDIS price guide guidelines and these prices are GST inclusive and include the cost of providing supports.
The price per hour are as follows and are guided by the NDIS gazetted rates:
NDIS Rate
Physiotherapy: $193.99/hr
*Please note that all pricing is subject to change which is pending the annual review of the NDIS price guide as of July 1st each financial year. Prices will subsequently be indexed accordingly.
Travel Charges
Travel charges are determined by the NDIS guidelines and are charged at 5 minute increments which is based on the therapists hourly rate as above. The maximal charge is 30 minutes of travel, but generally Home Therapy Australia PTY LTD will try and split the therapists travel time with other clients who may live in a proximal geographic location.
Additional Charges
Other expenses are the responsibility of the participant and/or the participant's representatives and aren't included with the cost of the supports provided. This includes cost to purchase equipment, entrance fees and tickets, meals etc.
Cancellation Policy
Participants must give 48 hours notice if they cannot attend a scheduled appointment. If the participant fails to give this notice, they will subsequently be billed for the support from their NDIS plan.
Payments
Home Therapy Australia will require payment for the provision of supports after supports are delivered.
Payments will be invoiced and applied depending on how the client's funds are managed:
If funds have been exhausted, or plans have expired, the participant acknowledges they are liable for payment of the services provided. Home Therapy Australia PTY LTD often does not have visibility over the allocation of funds including its' usage to manage budgets.
Changes to Terms of Service
If there are any changes to the supports that Home Therapy Australia PTY LTD provides and is delivered, the parties agree to discuss this and review the service agreement. Changes to the Service Agreement will thereby be in writing and signed and dated by all parties.
Ceasing Services
If any party wishes to end service delivery, they must provide at least one months notice. If there is a serious breach with either party, these requirements of notice will be waived.

- If NDIS funds are self managed by either the participant or a plan nominee, after providing the relevant supports, Home Therapy Australia PTY LTD will send the participant/nominee an invoice for the provided supports for the participant/nominee to pay. The participant/nominee will then pay the invoice by eft/bank transfer within 2 days. We encourage payment to be taken at the end of sessions through mobile eftpos which our clinicians are able to facilitate.
- If NDIS funds are plan managed, Home Therapy Australia will claim payment from the plan manger provider for the supports provided. It is essential that the Plan Manager details are provided prior to the scheduled appointment. Payment terms will be 7 days after issuing the invoice.
- If NDIS funds are NDIA managed, Home Therapy Australia PTY LTD provides NDIS supports (NDIS registered provider ID: 4050107713) who will claim funding as per the service provisions and service agreement made between the participant/guardian and Home Therapy Australia PTY LTD.
